Because that is how the American "Health System" is supposed to work. Right or wrong, that's what it is supposed to be. Do we even have a health system now? This isn't reported on, but many hospitals in less populated areas are closing down. Doctors and Nurses and other staff being laid off or "Furloughed," whatever that means. There were hospitals in rural areas, but many of them are either downsizing or closing outright because they couldn't make a profit off them, unlike hospitals in more urban areas. This especially goes for rural hospitals being absorbed by larger hospital chains. For an example, Google "Mayo hospital Dover-Foxcroft" (not related to the famous Mayo Clinic).

Because they really can't enforce it. There really isn't a way to do it. I was pulled over by Saginaw County shortly after the original EO was set forth. I've never known in any cop in my life to pull someone over for 4 mph over the speed limit. But I know it happens.
The officer asked where I was going, I told him McDonalds (as I truly was headed to the drive-thru). He simply said 'ok' and I asked him why he simply believed me. He said that they have to go on a person's word. No way to prove otherwise.

Because of reimbursement rates. At one time three types of hospitals got 100% rates: 1. rural, 2. urban (Black), and 3. teaching hospitals. Urban hospitals not affiliated with a teaching program, for the most part, really sucked at their levels of care. So the feds gave them a few years warning in the 90s to find a teaching program to take them on, and then cut the ones who couldn't sign onto a program down to the 60% rate.
Next up were the rural hospitals. I could write a lot about it, but the feds started with a mileage travel time rule with a bunch of wavers, then cranked down over time to squeeze more and more out down to 60%.
The body that controls all this is the former Center of Medicare and Medicaid Studies, now renamed the Centers for Medicare & Medicaid Services, but abbreviated CMS. They pick winners and losers, in particular for rural hospitals, and allow the politicians to keep their hands clean because they are 2 steps removed from Congress and politically appointed senior leaders.
CMS picks rates based on shadowy internal procedures, and regularly manipulates the levels of reimbursement to the hospitals in favor or out of favor (up to 50% for some procedures, on top of the travel time rules).
